LL-37 antimicrobial peptide — immune support or immune system overload?

I’ve been researching LL-37 for immune support since I seem to catch every cold that goes around my office. My functional medicine doc mentioned it as a possibility but I’m a bit confused about whether it actually helps your immune system or if it could potentially overstimulate it?

From what I understand, LL-37 is naturally produced in our bodies and has antimicrobial properties, but I’ve seen some forum posts elsewhere suggesting that taking it as a supplement could cause inflammation issues in some people. I’m already on low dose naltrexone for autoimmune stuff (Hashimoto’s) so I don’t want to do anything that might trigger a flare.

Has anyone here used LL-37 specifically for immune support? Did you notice actual improvements in not getting sick as often, or did anyone experience negative effects? Also curious about dosing – I’ve seen everything from 2mg to 10mg mentioned in different places.

I know everyone’s different and I should talk to my doctor (which I will) but wanted to hear real experiences first. Thanks in advance!

  1. I used LL-37 last winter for about 3 months, 5mg twice a week. Honestly didn’t notice a huge difference in terms of not getting sick – still got one bad cold in January. BUT I will say when I did get sick it seemed to resolve faster than usual, like 4-5 days instead of the typical week+ I usually deal with.

    The inflammation concern is real though imo. I have a friend who tried it and said she felt more joint achiness, but she also has rheumatoid arthritis so that might’ve been why. Since you already have Hashimoto’s I’d definitely start really low if you try it and monitor closely. Maybe even get some inflammation markers checked before and after?

  2. Interesting that BiohackerMike mentioned the faster recovery time because that’s exactly what I experienced too. I’m 38F and started LL-37 about 6 months ago, currently doing 3mg 3x per week. I wouldn’t say I get sick LESS often necessarily, but the duration is def shorter and symptoms seem milder.

    Re: the autoimmune concern – my naturopath explained that LL-37 is more about modulating the immune response rather than just ramping it up, which is different from some other immune boosters. But with Hashimoto’s you really need someone who understands both peptides AND autoimmune conditions to guide you. The LDN combo is something I’d be cautious about without professional oversight tbh.
