Knee replacement vs. regenerative alternatives — an honest comparison

So I’m 58F and been dealing with pretty bad osteoarthritis in my right knee for about 3 years now. Ortho surgeon says I’m a candidate for total knee replacement but honestly the recovery time and risks have me scared. I’ve been researching exosome therapy and PRP as alternatives and I’m super curious if anyone here has actually tried regenerative options before going under the knife?

My knee pain is like a 7/10 most days, worse going downstairs. I can still walk but not long distances anymore. Lost 35 lbs on tirzepatide which definitely helped but the joint damage is still there according to my MRI. Insurance obviously covers the replacement but I’d have to pay out of pocket for exosomes (looking at around $5-7k from what I’ve researched).

Just want to hear real experiences from people who’ve been in this situation. Did regenerative therapies actually help enough to avoid or delay surgery? Or am I just throwing money at something that won’t work for severe arthritis? My surgeon is pretty dismissive of anything that’s not surgery but I feel like I should at least explore all options first.

  1. I was in almost your exact situation 2 years ago at 56. Tried exosome injections (3 rounds, about 6k total) and tbh it gave me maybe 6 months of moderate improvement but didn’t last. Pain went from a 8/10 down to maybe a 5/10 but crept back up. Eventually did the knee replacement last May and I wish I’d just done it sooner. Recovery sucked for sure, took about 4 months to feel normalish, but now at 14 months post-op I’m hiking again. That said, my cartilage was completely gone, bone on bone. If yours isn’t that bad yet the exosomes might buy you more time?

  2. I’d get a second opinion from a doctor who actually does regenerative medicine before making the call. Most orthopedic surgeons only know surgery, that’s what they do. There are sports medicine docs and interventional orthopedists who specialize in biologics and can give you a real assessment of whether you’re a candidate. From what I’ve read, exosomes work best when there’s still some cartilage left to work with. If you’re bone-on-bone like the previous poster mentioned, you might be past the point where regenerative stuff can help enough. But def worth a consult with someone who knows both options.

  3. Have you looked into BPC-157 and TB-500 peptides alongside the exosome therapy? I know someone (not knee replacement level but bad meniscus tear) who did a combination protocol with exosomes, PRP, and those peptides and had really good results. It’s not a miracle cure but seemed to amplify the effects. Also way cheaper than multiple rounds of exosomes if you go the peptide route. Just another thing to research. Obviously talk to a doctor who understands peptides though, not all of them do.

  4. Thanks everyone, this is really helpful. MovementMatters47 I think my MRI showed I still have some cartilage but it’s definitely thinning in spots. Not completely bone on bone yet which is why I’m wondering if now is the time to try something before it gets worse. RegenMed_Curious you’re right, I should find a doc who does both and can give me an honest assessment instead of just asking my surgeon who only does replacements. Gonna look for a sports medicine consult in my area.
