Filler migration — TikTok horror stories vs. what dermatology research actually shows

Ok so I’ve been down a rabbit hole watching these TikTok videos about filler migration and honestly I’m freaking out a bit. I got .5ml of filler in my lips about 8 months ago and now I’m seeing all these posts about how filler migrates to weird places and creates that pillow face look even years later. But then I tried looking up actual research and the studies seem way less dramatic? Like they mention migration but not to the extent these influencers are claiming. I’m 34F and was thinking about getting a bit more filler in my cheeks but now I’m second guessing everything. Has anyone here actually experienced migration or is this just fear mongering for views? My injector is a dermatologist and she said the horror stories are mostly from people going to med spas with inexperienced injectors or getting way too much product at once. But idk, some of these before/after comparisons on social media are really convincing. Would love to hear from people who’ve had fillers long term and what your actual experience has been.

  1. The research on this is actually pretty interesting. Migration does happen but it’s usually minimal when done correctly – we’re talking millimeters, not the dramatic shifts you see on TikTok. The studies I’ve seen show that heavier fillers and overfilling are the main culprits. Also placement matters a lot. Your derm is right that technique makes a huge difference. The pillow face thing is usually from accumulation over time when people keep adding more without dissolving old product. That said, MRI studies have shown filler can persist way longer than we originally thought, so there is something to be cautious about.

  2. I had lip filler for about 3 years and honestly never had migration issues, but I was super conservative with amounts. The problem with TikTok is that extreme cases get the most views right? Nobody’s posting videos like ‘got subtle filler, looks normal, no problems’ lol. That doesn’t go viral. I will say though, after starting tirz for weight loss I noticed my face changed so much naturally that I ended up dissolving all my filler anyway. Just didn’t need it anymore with the fat loss in my face.

  3. One thing the studies show that TikTok doesn’t talk about is that migration is more common with certain products and certain areas. Like tear trough filler has higher migration rates than lips or cheeks. Also the whole ‘filler migrates to your lymph nodes’ thing has been documented but it’s not causing the aesthetic issues people think it is. What DOES cause problems is when injectors don’t understand facial anatomy and inject into the wrong tissue planes. Your derm sounds knowledgeable tbh. Maybe ask her specifically about the type of filler she uses and why she chose it?

  4. I think there’s truth somewhere in the middle. Yes TikTok exaggerates but also the aesthetics industry def downplayed migration risks for years. I’ve had filler in my cheeks for 18 months and haven’t noticed issues but I also only got 1ml total and haven’t gone back for more. My injector actually told me to wait at least a year between sessions which seems more conservative than what I see advertised at a lot of places. The before mentioned point about accumulation makes sense to me – like people getting touched up every 6 months for years, that adds up even if each session seems small
