I haven’t lost a pound in 6 weeks on the max dose. Is the medication broken?

I’ve been on tirzepatide for 4 months now and worked my way up to the max dose of 15mg. The first two months were amazing – I lost 22 pounds and felt like this was finally the answer. But now? Nothing. The scale hasn’t budged in 6 weeks and I’m honestly starting to wonder if something is wrong with the medication itself.

I’m doing everything the same as before. Same eating habits, same activity level, tracking my calories just like I was when it was working. I even switched to a new vial thinking maybe the first one went bad or something, but still nothing.

Has anyone else experienced this? Can these medications just stop working? I’ve read about people losing weight consistently for months and here I am stuck at the same weight for almost 2 months. My doctor just says to “keep at it” but I’m getting really discouraged. I’m wondering if I should switch to semaglutide instead or if I’m just wasting my time and money at this point.

  1. This is totally normal and not the medication being broken – you’ve hit a plateau which happens to almost everyone on GLP-1s. Your body adjusts and sometimes you need to reassess your calorie intake because as you lose weight your metabolism needs less fuel. What worked at 22 pounds heavier might now be your maintenance calories. I’d suggest tracking everything super carefully for a week and maybe reduce calories by 100-200. Also are you doing any resistance training? That made a huge difference for me when I stalled out.

  2. I feel you on this!! I stalled for almost 8 weeks on sema and wanted to scream lol. But then suddenly I dropped 4 pounds in like 10 days. My doctor mentioned that weight loss isn’t linear and our bodies do this weird thing where they hold onto water and then whoosh it all comes off at once. Also hormone fluctuations can play a role if you’re female – I notice I retain water at certain times of the month which makes the scale lie to me

  3. Mike’s right about the calorie adjustment but I’d also add – are you measuring other things besides the scale? Sometimes body recomposition happens where you’re losing fat but gaining or maintaining muscle, especially if you added any exercise. Take measurements and progress photos. I was “stuck” for 5 weeks but lost 2 inches off my waist during that time. The scale is just one data point and tbh not always the most accurate one for what’s actually happening with your body composition.
