PRP vs exosomes for joints — what does the evidence actually show?
So I’ve been dealing with chronic knee pain for about 2 years now (old basketball injury that never healed right) and my orthopedist mentioned both PRP and exosomes as options. He said exosomes are “the next big thing” but honestly they’re way more expensive and I’m trying to figure out if there’s actual evidence they work better than PRP or if it’s just hype.
I’ve done some googling but most of what I find is either from clinics trying to sell me treatments or super technical research papers I can barely understand. From what I can gather, PRP has been around longer so there’s more data, but exosomes supposedly have stronger anti-inflammatory effects?
Has anyone here done either treatment for joint issues? Did it actually help? My doc quoted me like $1500 for PRP vs $3500 for exosomes and I need to decide if the extra cost is worth it. Insurance covers neither obviously lol. I’m 38, pretty active, just want to get back to playing pickup without limping for 3 days after.
I did PRP in my shoulder about 18 months ago and honestly it was life changing for me. Took about 6 weeks to really notice improvement but by 3 months I had like 80% pain reduction. My doc said exosomes are promising but the research is still pretty early compared to PRP which has years of clinical data backing it. If money is tight I’d probably start with PRP since it’s proven to work for a lot of people. You can always do exosomes later if PRP doesn’t cut it
The mechanism is different though which is important to understand. PRP is basically using growth factors from your own blood to promote healing, while exosomes are cell-signaling particles that can modulate inflammation and potentially regenerate tissue more directly. I read somewhere that exosome quality varies WILDLY between providers though – like some are using plant-derived ones which probably don’t do much, others use mesenchymal stem cell derived exosomes which are supposed to be the good stuff. Did your ortho say where their exosomes come from? That might matter more than PRP vs exosomes tbh
I’m going through this exact decision right now for my hip! My doctor basically said what MitoMike said – PRP has way more evidence and predictable results. He’s seen exosomes work really well for some patients but it’s less consistent. One thing he mentioned is that sometimes they’ll combine both treatments for really stubborn cases. Maybe ask your doc if that’s an option? Though I’m guessing that would be even more expensive unfortunately.
Just wanted to add that I went down a pubmed rabbit hole on this last month. There are some promising small studies on exosomes for osteoarthritis and cartilage repair, but Sarah’s right that standardization is a huge issue. PRP at least has somewhat standardized protocols even if results vary. The other thing is most exosome studies are really small – like 20-30 patients – whereas PRP has been studied in thousands at this point. Not saying exosomes don’t work but the evidence base just isn’t there yet to justify double the price imo. Definitely something to watch though as more data comes out
