TRT can shut down sperm production — but is the fertility damage always reversible?

So I’m 35M and been on TRT for about 18 months now (150mg/week test cyp). My levels went from 280 to around 850 and I feel amazing — energy is back, gym gains are solid, mood is way better. But here’s the thing… my wife and I weren’t planning on kids anytime soon but now we’re actually talking about it seriously and I’m getting worried. My doc mentioned at the start that TRT shuts down your natural production and can tank sperm count, but he made it sound like it wasn’t a huge deal and things would bounce back. Now I’m reading conflicting stuff online about guys who came off TRT and their fertility never fully recovered. Some guys seem to bounce back in a few months with HCG, others are saying they’ve been off for over a year with no luck. Does anyone here have actual experience coming off TRT and trying to conceive? Did your sperm count recover? How long did it take? I’m trying to figure out if I should come off now preemptively or if I’m panicking over nothing. Really don’t want to mess up our chances of having kids but also don’t want to go back to feeling like garbage if I don’t have to yet.

  1. I went through this exact situation last year man. Was on TRT for 2 years, came off to try for baby #2. My urologist put me on HCG and Clomid together and it took about 5 months before my sperm count was back in normal range. Not gonna lie, those months off TRT sucked energy-wise, but we did end up conceiving at month 6. The key thing my doc said was that the longer you’re on, the harder the recovery can be. 18 months isn’t terrible though. Definitely get a sperm analysis done first before you make any decisions, you might still have some swimmers even on TRT. Some guys do.

  2. tbh you should’ve been taking HCG alongside your TRT from the beginning if fertility was ever going to be a concern. That’s what keeps your boys functioning even while on testosterone. I’m 38 and been on TRT for 3 years but I do 250iu HCG 3x per week and my doc says my testicular function should be maintained. Haven’t tried to conceive yet so can’t say for sure but the theory is sound. Might be worth asking your doc about adding HCG NOW rather than coming off completely, then when you’re ready to try you’ll be in better shape.

  3. Just wanted to chime in with a different perspective. I was on TRT for only 10 months and came off completely 8 months ago. Still waiting for things to normalize fully. My sperm count is back but total T is hovering around 350-400, so better than before TRT but not great. My fertility doc said younger guys and shorter duration usually recover better, so you might be fine. But the guys saying everyone bounces back easy are not telling the whole story imo. Some of us get stuck in a weird middle ground. MaxPower_81’s approach with HCG and Clomid seems like the smart medical way to do it rather than just cold turkey.

  4. The research on this is pretty clear that MOST men recover fertility after stopping TRT, but it can take 6-24 months and some percentage don’t fully recover. The variables are age, duration of use, dosage, and individual biology. At 35 with 18 months use, you’re probably in a decent position but there’s no guarantee. I’d recommend: 1) get a current semen analysis to see where you stand NOW, 2) talk to a reproductive endocrinologist not just your TRT doc, and 3) consider HCG protocol either added to TRT or as part of a restart protocol. Don’t just wing this based on forum advice when your future family is on the line.
